12-Month Questionnaires The Ages & Stages Questionnaires: The ASQ format has been used since 1980 to provide comprehensive preliminary screenings of large groups of infants and young children, as well as to monitor the development of children who may be at risk for developmental delays or disabilities. We distribute the ASQ to participating families at several selected age intervals; at the 12-month inverval, the appropriate ASQ and Macarthur-Bates CDI (explained below) are mailed to each family with a stamped and return-addressed envelope. Each ASQ questionnaire can be completed within 10-15 minutes.
The MacArthur-Bates Communicative Development Inventories (CDI): The CDI became widely used as a standardized measure of language and communication skills in infants and young children in the 1990s. The CDI: Words and Gestures (Infant form) is designed for use with 8- to 16- month old children, and is distributed along with the ASQ to participating families at 12- months. Each CDI questionnaire can take anywhere from 5-25 minutes to complete, depending on how many words your child understands and/or speaks.
